Responsible for supporting the delivery of health and social care functions for adults/carers for the people of Wirral. A key role in supporting delivery of the Personalisation and Inclusion Pathways for adults and carers ensuring the legal framework of the Care Act 2014, strength and asset-based practice, personalisation and social inclusion is embedded and applied effectively. Assessing, accessing and implementing a range of social care services and providing guidance/advice and information for adults with acute or increasingly complex long-term physical health, mental health needs and for older people and carers. Providing supervision and practice support to AYSE (Assessed & Supported Year in Employment) /newly qualified social workers and Care Navigators within the service. Wirral are using the 3 Conversations model (3C’s) across all Adult Social Care teams. The ethos of the model is to identify individual’s strengths and aspirations to maximise their independence and wellbeing. Integrated Community Coordination Teams – Whilst working in the ICCT’s you will be completing strengths-based Care Act assessments and person centred support planning with a wide variety of service user groups aged 18 and over. The work is diverse and the aim is to promote the wellbeing of adults with care and support needs to continuously improve their outcomes, creating lasting connections with their local community. Multi Agency Safeguarding Hub – The MASH team are a well-established Social Work team supporting vulnerable adults who are experiencing or at risk of abuse or neglect. The role of MASH is to screen and complete Safeguarding S42 enquiries, working closely with the individual and our partner agency. Review – The Review Team covers a wide remit of reassessment across the Wirral, those recently discharged from hospital and people residing in the community. In addition, working alongside contracts & commissioning and quality improvement teams, adhering to outcomes focused and legal literate practices. Urgent Community Response- UCR – Work collaboratively across a multi-disciplinary team, which includes Nurses, Therapists, GP¿s and Community Geriatricians. Opportunities include- Home First, Urgent 2-hour Community Response, Frailty Virtual Ward. The Successful candidate will have a key role to play in supporting the delivery of the Personalisation and inclusion Pathways for Adults and Carers ensuring legal framework of the Care Act 2014. Working Hours: 36 hours per week, various shift patterns to cover 8am-8pm (weekends 9-5pm on a rota basis) Deprivation of Liberty Safeguards (DoL’s) – Experienced Social Workers are required to work within a small team of Best Interest Assessors (BIA) within the Council. The BIA will undertake DoLS assessments of individuals who are, or may become deprived of their liberty. The BIA will also support with the duty function and provide advice and guidance to other BIA’s across Adult Social Care when required. Professional Standards – The Professional Standards Team deliver an essential role to ensure the quality of practice across the Adult Social Care directorate adheres to practice frameworks. The team provide assurance relating to quality of interventions through a series of audit processes.
