An opportunity has arisen for Band 4 rehabilitation assistant to join the Reablement Therapy team that covers the MFFD wards at YDH and Local bedded units as part of our 7 days working expansion.

Enjoy rehabilitation and enabling patients to achieve there goals?

If so then this is the exciting opportunity for you, we work with patients to achieve their rehabilitation goals and enable them to return back to their normal place of residence where possible using community services such as D2A, Community SPOC and IRT as needed.

Main duties of the job The successful candidate will have relevant experience in the acute or community setting preferably within rehabilitation.

You will have very good verbal and written communication and interpersonal skills.

You will be able to demonstrate the ability to work autonomously, as well as part of the therapy and wider MDTs to ensure our patients are receiving the best service possible.

You will need to have a good awareness of your skill set and be aware of when you need to seek qualified support.
