The service is a bespoke specialist service for people with spectrum conditions & associated complex needs. You will be assisting individuals through a care pathway to help them live as independently as possible. You will be helping with their Progression through a care pathway to live as independently as possible. You will have the full support of a team of multi-disciplinary professionals.
