Band 6 - Community Mental Health PractitionerSearch Healthcare are recruiting an experienced Community Mental Health Practitioner (Band 6) on behalf of our client on an initial, 3 month contract in a Monday- Friday, 9am-5pm position.

Position Overview:Delivering recovery-focused interventions and coordinating care for service users in the community, including CPA care coordination and managing clients with enduring mental health needs within a caseload of 25.

Prioritising and managing caseloads collaboratively with health professionals, utilising strong communication skills and a non-judgemental approach.

Conducting mental health and risk assessments to inform medical decisions, completing Care Act Assessments, and facilitating home visits for holistic support.

Referring patients back to primary care or other health professionals as needed, while utilising IT and multidisciplinary team channels for efficient patient screening and coordination.

Providing personalised support to individuals, families, and carers, empowering them to manage their health independently and achieve personalised care plan objectives.

Collaborating with existing services to connect patients to community activities that promote health and wellbeing autonomy.

Requirements:-A full UK Driving Licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010-The right to work in the UK-Minimum 6 months experience in a UK based healthcare setting.

-current registration with the NMC and adheres to the NMC Code.

-Professional qualification in a relevant field-Experience in mental health settings, addressing complex community issues with service users.
