Care & Support WorkerHarrow£25,642.50Overall ResponsibilityWithin this role you will support residents with enduring mental health issues. This can be highly challenging but rewarding in equal measure. As a key worker you will be fully responsible for your customers? support plans/risk assessment and encouraging their independence. The customers? all have complex enduring mental ill health, regularly the result of drug induced psychosis, with unpredictable behavioural patterns. However they are living in self contained flats and encouraged to manage their own well-being. They require positive support to ensure their independence is maintained and they are encouraged to remain as active in the community as possible. The ultimate objective if for residence to return to independent living in the community.Key responsibilitiesEmpower customers to maintain and establish positive relationships in the wider communityProvide practical support to customers for essential daily living tasksEncourage customers in developing and maintaining independence and emotional wellbeingEnsure customers are supported in a safe and caring environment with the confidence to raise issuesRecognise changes in customer behaviours and potential safeguarding issues associated. Respond and challenge in an appropriate and empathetic mannerWork with colleagues to maintain accurate customer recordsWork closely and cooperatively with external agencies and family members to offer customers a cohesive care and support service to develop and/or maintain independenceUndertake risk assessments for customers and administration and recording of medication in line with care plansDe-escalate challenging behaviours in a way that respects customers whilst showing respect and limiting any impact on other customers or colleaguesGeneral responsibilitiesTo understand and comply with MTVH?s safeguarding, safety and compliance policies, procedures and processes and support a positive safeguarding, safety and compliance culture across the organization.Actively and effectively, promote the organisations corporate values of Care, Dare, Collaborate.
